By preregistering online, you don’t have to arrive early for your
appointment to fill out the forms in the office, in addition to having
the forms neatly typed and printed out.
After filling out the online forms, please click the ‘Submit’ button
at the bottom of the page to automatically submit your information to
our office.
The forms that you complete and submit online will be made available
for your signature when you arrive at our office for your appointment.
Our primary concern is the security and privacy of your personal
information and we would like to reinforce that we have taken all
measures to protect it.
Technical Note
PC Users
Our Online-Form uses Adobe Acrobat Reader 5 or greater plug-in, so
that you can conveniently submit your Online-Form, from the comfort of
your home or work.
If Adobe Acrobat Reader 5 or greater is not already installed on
your computer, please click the link on the Forms Main Page to download
the free plug-in from Adobe's web site.
It is important that you have at least version 5 of the plug-in to successfully use our online form.
Mac Users
You must open and submit the form in the latest version of Apple's Safari Browser on Mac operating system.
It is also important to have the latest version of Adobe Acrobat
Reader on your Mac in order to submit your form to our office correctly.
Please download the free plug-in from Adobe's web site if Adobe
Acrobat Reader 5 or greater is not already installed on your MAC.
